At 12 weeks pregnant you are about 3 months along, near the end of the first trimester, with about 28 weeks to go until your estimated due date.

At 12 weeks pregnant your baby has a more defined profile and tiny fingernails, and you may be wondering when the nausea will let up, whether you should be showing, and what the 12 week scan actually checks.

This guide covers development, size, symptoms, your belly, screening tests, sleep and the signs that mean you should call. Every clinical detail was read in October 2026 on the primary sources named in the text, and where they disagree we print both.

Key Takeaways

  • 12 weeks pregnant is about 3 months, or 10 weeks after conception (Mayo Clinic), with about 28 weeks to go.
  • Week 12 is in the first trimester by the ACOG definition UPMC uses, by the Office on Women’s Health and by Mayo Clinic.
  • Mayo Clinic puts the baby at about 2 1/2 inches crown to rump and 1/2 ounce. Nemours KidsHealth compares it to a small lemon or lime.
  • cfDNA screening can start at 10 weeks and CVS is usually done between weeks 10 and 13 (MedlinePlus). The first trimester screen is around weeks 11 to 13 (UPMC).
  • Bleeding, severe belly or pelvic pain, shoulder pain, fainting, severe vomiting or a fever of 100.4 F are reasons to call right away.

Quick Answer

At 12 weeks pregnant you are about 3 months along, with about 28 weeks to go, near the end of the first trimester. Mayo Clinic puts the baby at about 2 1/2 inches from crown to rump and about 1/2 ounce. The first trimester screen is done around weeks 11 to 13 (UPMC).

12 weeks pregnant: quick facts with sources.

How the weeks are counted. The Office on Women’s Health says pregnancy lasts about 40 weeks, counting from the first day of your last normal period. Mayo Clinic explains that conception typically happens about two weeks after that period begins, which is why it describes week 12 as 10 weeks after conception. Nemours KidsHealth notes that an early ultrasound can help set your due date, so your dates may be adjusted.

Your Baby’s Development at 12 Weeks

These are the changes the references tie to week 12. Every baby develops on its own timeline.

Source and timingWhat it says is happening
Mayo Clinic, week 12 (10 weeks after conception)Fingernails are sprouting, the face has a more developed profile, and the intestines are now in the abdomen.
Office on Women’s Health, at 12 weeksNerves and muscles begin to work together, and the baby can make a fist. The external sex organs show. The eyelids close and will not open again until the 28th week. Head growth has slowed and the baby is much longer.
Nemours KidsHealth, week 12The brain keeps developing and fingernails and toenails start to form. Around now the vocal cords form and the kidneys start working: the baby can pass swallowed amniotic fluid as urine.
Fetal development around week 12, by source.

Can you tell the sex yet? OWH says an ultrasound in the second trimester or later might be able to show it. UPMC says the noninvasive prenatal test (NIPT), a blood test, can reveal the sex. MedlinePlus says cell free DNA screening may be used to determine sex, usually when an ultrasound shows the genitals are not clearly male or female. Finding out is your choice.

How Big Is Your Baby at 12 Weeks?

SourceWeekLengthWeight
Mayo Clinic12About 2 1/2 inches (61 mm), crown to rumpAbout 1/2 ounce (14 g)
Nemours KidsHealth122 to 2 1/2 inches, crown to rump; “about the size of a small lemon or lime”Not given
Office on Women’s Health12About 3 inchesAlmost an ounce
UPMC10 to 13Grows from about one to three inchesNot given
Baby size at 12 weeks. Sources differ, so each is printed.

Mayo Clinic measures crown to rump, from the top of the head to the bottom of the tailbone, so the legs are left out. OWH does not say how it measures, which may be why its figure is longer. These are typical figures, not targets. The lemon or lime comparison is from Nemours KidsHealth, the only primary source we read that offers one for week 12.

12 Weeks Pregnant Symptoms

These first trimester changes come from the Office on Women’s Health list. Having many, few or almost none can all be normal.

SymptomWhat the sources sayWhat may help
NauseaOWH says you might notice nausea going away in the second trimester. Nemours KidsHealth says at week 12 morning sickness may not fully go away yet.Smaller meals more often (UPMC).
TirednessNemours KidsHealth says your energy should soon start picking up again. An afternoon nap and short walks outdoors (UPMC).
Tender breastsUPMC says they may be sore as they prepare for breastfeeding.A supportive, comfortable bra.
Peeing more oftenNemours KidsHealth links it to your expanding uterus.Fewer fluids close to bedtime (MedlinePlus).
ConstipationOn the OWH list.More water and fiber (UPMC).
Mood swingsOn the OWH list.Talk to a friend or therapist, meditate, do yoga or walk (UPMC).
Common symptoms at 12 weeks pregnant.

OWH also lists headache, heartburn, food cravings or distaste, and weight gain or loss. Nemours KidsHealth adds temporary acne, gums that may bleed when you brush and dark skin patches.

  • Spotting: UPMC says it can be normal in the first trimester but can also be a sign of a miscarriage or an ectopic pregnancy. Tell your provider about any bleeding.
  • Discharge: UPMC says green or yellow discharge, or discharge with a bad odor, could mean an infection.
  • Headaches and dizziness: the CDC lists a headache that will not go away or gets worse, and dizziness or fainting, as urgent warning signs.

Your Body and Belly at 12 Weeks

Will you show at 12 weeks? Maybe, maybe not. None of the primary sources we read gives a week by which you should be showing, so a flat belly, some bloating or a small bump can all fit week 12. Nemours KidsHealth says your uterus is expanding, and OWH lists weight gain or loss as a first trimester change.

Sharing the news. Many people wait until after the first trimester to tell others, and many share earlier. It is a personal choice, not a medical rule. UPMC notes that symptoms can be harder to deal with while you keep the pregnancy a secret, so telling one person you trust can help.

Prenatal Care, Tests and Appointments Around Week 12

UPMC says first trimester visits are usually about once a month, with the second visit around weeks 11 to 13, when you will likely have your first ultrasound. If your first visit has not happened, Nemours KidsHealth says to schedule it this week or as soon as possible: your provider will estimate your due date, do a physical exam and set up a care plan, and offer prenatal blood tests.

TestTimingWhat it isSource
First trimester screenAround weeks 11 to 13A blood test plus an ultrasound measuring fluid at the back of the baby’s neck, to estimate the chance of conditions such as Down syndrome and trisomy 18UPMC
Cell free DNA (cfDNA) screening, or NIPTAs early as the 10th weekA blood test most often used to show an increased chance of trisomy 21, 18 or 13MedlinePlus
Chorionic villus sampling (CVS)Usually weeks 10 to 13A placenta tissue sample; used to confirm a diagnosisMedlinePlus
AmniocentesisUsually weeks 15 to 20 (later)An amniotic fluid sample; used to confirm a diagnosisMedlinePlus
Tests around 12 weeks, with each source’s timing (read October 2026).

Screening vs diagnostic. MedlinePlus says cfDNA screening cannot tell you for sure whether your baby has a chromosome disorder: a positive result means an increased chance, and confirming a diagnosis takes a test such as CVS or amniocentesis. UPMC says first trimester screening is optional and positive results need a diagnostic test.

Screening is your choice. Since the first trimester screen window ends around week 13, ask now if you want it. Hearing the heartbeat: whether your provider listens at this visit, and how, varies, so ask what to expect, and talk with your provider before buying a home heartbeat monitor.

What to Do This Week: A Simple Checklist

  • Book or confirm your visit: Nemours KidsHealth says to schedule your first prenatal visit this week if you have not.
  • Decide about screening: write down your questions about the tests above.
  • Keep taking your prenatal vitamin: UPMC advises a prenatal vitamin with folic acid daily to support normal brain and spine development. See our prenatal vitamins guide.
  • Move gently: UPMC suggests short walks for fatigue. Ask your provider about adjusting any workout.
  • Save your call list: your provider’s daytime and after-hours numbers, plus the warning signs at the end of this guide.

Sleep at 12 Weeks Pregnant

MedlinePlus says you may sleep well during the first trimester and may need more sleep than usual. OWH suggests adjusting your routine, such as going to bed earlier, and UPMC suggests an afternoon nap.

Sleep position. None of our sources sets a position rule for week 12. MedlinePlus’s general pregnancy advice is that lying on your side with your knees bent will likely be most comfortable, and that it keeps your baby from pressing on the large vein that carries blood back to your heart from your legs. It adds that many providers tell pregnant women to sleep on their left side. Ask your provider what they recommend for you.

  • Use pillows: under your belly or between your legs (MedlinePlus). See how to use a pregnancy pillow.
  • Watch the evening: MedlinePlus advises limiting soda, coffee and tea, and avoiding a lot of fluids or a big meal within a few hours of bed.

MedlinePlus says not to take any sleep aids, including over-the-counter and herbal products, without talking to your provider. Our guide to melatonin during pregnancy lists questions to ask.

For Partners: How to Help at 12 Weeks

  • Come to the appointment: the visit around weeks 11 to 13 often includes the first ultrasound (UPMC) and screening decisions.
  • Protect rest: pick up chores so there is room for a nap or an early night.
  • Learn the warning signs: below, so you can make the call if she is unsure.

Looking Ahead: What Comes Next

  • Week 13: OWH counts the second trimester from week 13, and the first trimester screen window (UPMC) closes.
  • Weeks 15 to 20: MedlinePlus says amniocentesis, if you choose it, is usually done now. OWH says most women find the second trimester easier than the first.

When to Call Your Provider

Call any time something worries you. UPMC lists these first trimester warning signs, and says to go to the emergency department if symptoms are serious: severe nausea and vomiting, vaginal bleeding or spotting, cramping, green or yellow or bad-smelling discharge, pain when you urinate, or a temperature above 100.4 F.

Ectopic pregnancy. Mayo Clinic says the first warning signs are often light vaginal bleeding and pelvic pain, and that leaking blood can cause shoulder pain or an urge to have a bowel movement. It says to seek emergency medical help for:

  • Severe abdominal or pelvic pain with vaginal bleeding.
  • Extreme lightheadedness or fainting.
  • Shoulder pain.

Seek medical care immediately for any of the CDC’s urgent maternal warning signs:

  • A headache that will not go away or gets worse, dizziness or fainting, or vision changes.
  • A fever of 100.4 F or higher.
  • Extreme swelling of your hands or face.
  • Trouble breathing, chest pain or a fast-beating heart.
  • Severe nausea and throwing up, or severe belly pain that does not go away.
  • Vaginal bleeding or fluid leaking, or overwhelming tiredness.
  • Thoughts about harming yourself or your baby.

For heavy bleeding, fainting, severe pain, chest pain or trouble breathing, call 911 or go to the nearest emergency room, and say you are pregnant. This article is general education, not medical advice.

Frequently Asked Questions

How many months is 12 weeks pregnant?

About 3 months. Twelve weeks is 84 days, close to three calendar months counted from the first day of your last period, using the convention of 4 weeks and a few days per month. Months are a convention, so your provider counts in weeks.

How big is my baby at 12 weeks?

Mayo Clinic says about 2 1/2 inches from crown to rump and about 1/2 ounce. Nemours KidsHealth gives 2 to 2 1/2 inches crown to rump and compares it to a small lemon or lime. The Office on Women’s Health lists about 3 inches and almost an ounce.

What symptoms are common at 12 weeks pregnant?

The Office on Women’s Health lists tiredness, tender breasts, nausea, peeing more often, constipation, mood swings and headache. Nemours KidsHealth says energy often starts picking up around now, though morning sickness may not fully go away yet. Severe vomiting or bleeding is a reason to call.

Should I be showing at 12 weeks pregnant?

There is no set week. None of the primary sources we read gives a week by which you should show, so a flat belly or a small bump can both be normal at 12 weeks. Nemours KidsHealth says the uterus is expanding.

What does the 12 week ultrasound check?

UPMC says the first trimester screen, around weeks 11 to 13, combines a blood test with an ultrasound that measures fluid at the back of the baby’s neck to estimate the chance of conditions such as Down syndrome and trisomy 18. It is optional, and a positive result needs a diagnostic test.

Is spotting normal at 12 weeks pregnant?

It can be. UPMC says spotting in the first trimester can be normal but can also be a sign of a miscarriage or an ectopic pregnancy, so report any bleeding. Mayo Clinic says to seek emergency help for severe abdominal or pelvic pain with vaginal bleeding, extreme lightheadedness or fainting, or shoulder pain.
